Health Care Financing:
MCHB Implementation Grants
MCHB has awarded 7 state
implementation grants. They are part of the 2nd core outcome
of the HP 2010 Performance Outcomes. It states that all
families of CSHCN will have adequate public and/or private
insurance to pay for the services they need. You can read
a brief portion of each grants purpose and goals as well
as access the grant abstract below.
Florida - Outreach to Uninsured Children with Special
Health Care Needs: Telemedicine Links Between Title V and
Community Health Centers

The purpose of this project is to link Florida's Title V
program (Children's Medical Services; CMS) and local community
health centers (CHC) - to reach uninsured CSHCN in Florida
and enroll them in insurance, particularly those in underserved
communities that traditionally have faced numerous barriers
to care: the black and Hispanic communities, and children
living in rural areas. Objectives include targeting outreach
efforts to low income families who have uninsured CSHCN;
to establish CHC screening and outreach activities; to implement
telemedicine linkages between CHCs and CMS; to target outreach
efforts to Hispanic and black communities using grandmothers;
and to review screening and enrollment in factors. Grant

Maryland - Maryland Family Access Project

The purpose of this project is to assist families to
access comprehensive services from MCOs which meet all needs
of their CSHCN population, and to ensure their enrollment
in the plan best suited to their individual needs. They
plan to train a cadre of parents of CSHCN to accomplish
the following: 1) work with other parents, MCOs, pediatricians'
offices, and state agencies to increase their understanding
of the needs of CSHCN and 2) ensure that public and private
managed care plans address the needs of CSHCN. Grant

Massachusetts - Massachusetts Initiative to Address
Adequate Health Care Financing for CSHCN

The purpose of this project is to establish the Massachusetts
Initiative to Address Adequate Health Care Financing for
CSHCN under the auspices of the Massachusetts Consortium
for CSHCN. The Consortium for CSHCN is a state level collaborative
forum for information exchange and policy consultation among
providers, families, health plans, policy makers and state
agency personnel. This Initiative will address specific
gaps in the health care financing system for CSHCN through
3 primary objectives: 1) develop a blended strategy for
financing enhanced care coordination; 2) increase the direct
participation of families in improving health care financing
systems; and, 3) develop and promote the adoption of a comprehensive
pediatric definition of medical necessity.

Abstract 
New Hampshire - Partners Enhancing Managed Care Expansion

The purpose of this project is to apply a new structured
primary care based model of Enhanced Care Coordination(ECC)
as a case management insurance benefit for CSHCN. This model
has been shown to work for health providers, insurers, and
families and to reduce health costs. The ECC model is family
centered, and is designed to provide comprehensive care
coordination in the primary care setting where the insurer
case manager collaborates closely with the practice &
other community & specialty care players to meet the
full range of needs of families with complex health conditions.
The purpose of this proposal is to develop statewide and
community level supports and training and then disseminate
this comprehensive care coordination approach through private
and public insurers for CSHCN in pediatric practices in
NH and then ME. Following evaluation of these expansion
efforts, the partners will focus on revising and sustaining
these services in pediatric practices. At the health plan
level this project will utilize the information that results
from this comprehensive partnership to assist the quality
assurance units in both Anthem Blue Cross Blue Shield and
Cigna in implementing ongoing quality assessment and quality
improvement initiatives to improve the care given to all
CSHCN. Grant

Virginia - Improving Access to Comprehensive Insurance
Benefits and Services for Children with Special Health Care
Needs

The Virginia Department of Health is the state's Title V
agency and is responsible for this project. Grant partners
include VDH's network of six regionally managed, community-based
Care Connection for Children Centers (CCC Centers); families;
the Department of Medical Assistance Services (Medicaid
Agency); the Department of Mental Health (Part C); the Department
of Education; insurers; hospitals, providers, equipment
suppliers, and employers.
The purpose of this project is to review, recommend, and
implement system changes to provide insurance to low income,
uninsured CSHCN. The CCC Centers will work with family and
community groups to enroll low income uninsured CSHCN in
public insurance programs; form partnerships with insurers
to demonstrate the value of providing TitleV funded wraparound
intensive, community-based care coordination services to
children enrolled in Medicaid HMO, FAMIS (SCHIP), and commercial
HMO plans; work with partners to develop a Model Benefit
Package (including medical, dental, and mental health coverage)
that is reasonable, affordable, and adequate; and recommend
system changes to remove barriers to care including referrals,
pre-authorizations, and reimbursement issues.Grant

Washington - Care Management for Children with Special
Health Care Needs: Addressing and Financing an Unmet Need

The purpose of this project is to develop, implement and
evaluate a quality care management program for children
with special needs in a large private health plan that includes
a Medicaid managed care population. The Project will pilot
the use of an identification system based on diagnostic
classification to stratify these children for care management
purposes in two demographically and geographically diverse
areas of the state. A care management program and strategies
for reimbursing care management activities will be implemented.
Integration of these concepts and practices in Regence BlueShield's
statewide system and the Medical Assistance Administration's
managed care programs for children with special health care
needs will occur.

Wisconsin - Covering Kids with Special Needs (CKSN)

ABC for Health (ABC) is a Wisconsin-based, public interest
law firm whose mission to help families and CSHCN link to
health care coverage programs through direct client services
to more than 1000 families each year, a very active educational
program including professional and family-oriented training
and publications, and policy initiatives based on collaborative
coalitions.
The purpose of this project is to expand coalition-building
efforts focused on CSHCN and to link with the larger coalitions
looking at access/ coverage/ and enrollment issues in health
care. Working with Wisconsin's Title V CSHCN Program and
their five Regional CSHCN Centers to initiate new local
HealthWatch committees, which will gather information on
issues facing CSHCN, provide resources and strategies to
address the issues, and promote networking and collaboration
between parents and a spectrum of service providers across
Wisconsin. Other objectives include: Promote public policy
strategies and initiatives to decrease uninsured and/or
increase coverage of underinsured; Providing direct health
benefits counseling services & tracking client encounters
to identify systemic problems; Creating training & educational
materials around the 2010 six core outcome measures. Grant
